Abstract. In a context of growing incidence of chronic diseases and ageing populations, there is the need to research and find new solutions to shift resources into the community in an effort to deal more effectively with chronic conditions. In that direction, patient empowerment plays an important role. Approaches to increase patient empowerment vary from patient self-management programs, to promoting patient involvement in treatment shared decision-making, to facilitating the clinician-patient cooperation. This talk focuses on self-management as a way to engage and empower patients in order to improve their quality a life and to allow a better follow-up by clinicians. Very preliminary results from the CONNECARE project are also presented to show how self-management could be put in practice for supporting complex chronic patients.


Eloisa Vargiu - Linedin

Eloisa Vargiu holds a Ph.D. degree in Electronic and Computer Engineering by the University of Cagliari (Italy). Currently, she is managing the Integrated Care research line at the eHealth R&D Unit, EURECAT. In April 2016, she started the technical coordination of the EU project CONNECARE (H2020). She is also technically coordinating from EURECAT two Spanish projects (myOSA and GERAR). From January 2012 to June 2015, she was the technical coordinator and main researcher of the EU project “BackHome” that was evaluated with "Excellent" by the EC.
Her research interests are on “integrated care”, “telemonitoring and home support”, “intelligent systems”, and “applied artificial intelligence”. She is co-author of more than 70 peer-reviewed publications in international journals and conferences, as well as co-editor of 4 research books.
